*Important* – All Religious Education Families must fill out an updated Nativity Religious Education Guidelines form before each Summer or Fall session. Please list all of your children’s names and grades on one form. Also, all families must fill out a Safe Environment for Children form each year, one per family. The Nativity Self Dismissal form must also be completed each year, one for each student, before both summer and fall sessions. You do not have to fill out a self dismissal form for GIFT/ home study students, but the Guidelines form and Safe Environment for Children form must still be filled out. Once all required forms are filled out, please upload each one into your Parish Giving account under Family Documents. Please click Documents then My Documents. After uploading, each document must be saved in order for it to be retained in the system. It is very important that you label the files for the documents as follows before uploading. Example- Smith 23-24 NG form, Smith 23-24 SE form, John Smith 23-24 SD form, Jane Smith 23-24 SD form, etc.
* 8th grade students only, please click the confirmation paperwork link below for additional required documents. If you do not have an 8th grade student fall 23-24, you may disregard the other links listed under Family Documents in your Parish Giving account as they are for Confirmation paperwork specific to the 8th grade.
